Location:     Wasatch Golf Course - Wasatch State Park - 975 Golf Course Dr, Midway, UT 84049

• 7:00 a.m. check-in, 8:00 a.m. shot-gun - tee-off
• 4-Person Scramble
• $100 per player or $400 for a four-some
• Mulligans - $5 ea (maximum of 8 per foursome)
• $100 to sponsor a hole – company name on the hole
• $500 to have a booth or table on the hole
• Lunch will be provided after golf
• Awards and raffle provided during the luncheon
